Types of Housing/Care Search Definitions
Adult Day Care Facilities Adult Day Care Day Activity and Health Services facilities provide daytime services, up to 10 hours per day, Monday through Friday, to individuals residing in the community.
Alzheimer’s/Memory Care Memory Care A Type B assisted living facility that is certified to provide specialized services to residents with Alzheimer’s or a related condition.
Assisted Living Facilities Assisted Living The Assisted Living facilities provide individualized health and personal care assistance in a homelike setting with an emphasis on personal dignity, autonomy, independence and privacy.
Affordable Senior Housing Affordable Senior Housing HUD 202 Program that offers rental assistance for seniors who meet the requirements of the federal program.
Home Health Agencies Home Care Home Health Agencies provide a wide range of health and social services delivered at home to persons recovering from an illness or injury, or persons who are elderly, disabled and/or chronically ill. Home Health Agencies provide “skilled services” such as nursing, social services, and therapeutic treatments (physical, speech, occupational therapy). They also provide non-skilled services like help with bathing, dressing and eating. Medical equipment such as wheelchairs, walkers and oxygen may also be provided.
Hospice Hospice Hospice Agencies provide intervention services that focus primarily on the reduction or abatement of physical, psychological, or spiritual symptoms of a terminal illness for consumers.
Nursing Facility/Homes Nursing The Nursing Facility program provides institutional care to Medicaid recipients whose medical condition regularly requires the skills of licensed nurses.
Personal Assistance Services Home Care This program contracts with licensed agencies to provide personal assistance services to mentally competent clients with physical disabilities who are willing to supervise their attendant or have someone who can supervise the attendant. To be eligible for this program you must meet the following criteria: age 18 or older, mentally and emotionally capable of directing your own care, and have a physical disability that is expected to last at least six months from the date eligibility is determined.
