Types of Housing/Care Search Definitions
Adult Day Services Adult Day Care Adult Day Services (ADS) are coordinated by a person or any legal entity which provides care and an array of social, medical, or other support services for a period of less than 24 consecutive hours in a community-based program to elderly or disabled persons who require or request such services due to age or functional impairment.
Affordable Senior Housing Affordable Senior Housing HUD 202 Program that offers rental assistance for seniors who meet the requirements of the federal program.
Assisted Living Facilities Assisted Living An Assisted Living Facility is any institution, facility, place, or building in which there are provided for a period exceeding 24 consecutive hours, through ownership, contract, or preferred provider arrangements, accommodation, board, and an array of services for assistance with or provision of personal care, activities of daily living, health maintenance activities, or other supportive services. Services are provided for four or more non-related, senior individuals who have been determined to need or want these services. Assisted living promotes resident self-direction and participation in decisions which emphasize independence, individuality, privacy, dignity, and residential surroundings. This definition does not include (a) those homes, apartments, or facilities providing casual care at irregular intervals and (b) those homes, apartments, or facilities in which a competent resident provides or contracts for his or her own personal or professional services if no more than 25% of the residents receive such services. A competent resident is someone who has the capability and capacity to make an informed decision.
Home Health Agencies Home Care A Home Health Agency is a public agency, private organization, or subdivision of such an agency or organization which is primarily engaged in providing skilled nursing care or a minimum of one other therapeutic service on a full-time, part-time, or intermittent basis to patients. Health care is provided in a place of temporary or permanent residence used as the patient's home under a plan of care prescribed by the attending physician.
Hospice Hospice Hospice is a coordinated program under the direction of an identifiable hospice administration which provides: home care and inpatient care, palliative care, supportive medical care, and any other care to meet the needs of terminally ill patients and their families, uses a medically directed hospice interdisciplinary team including services provided by trained volunteers, provides care to meet physical, psychological, spiritual, and other special needs which are experienced during the final stages of illness and during dying and bereavement, and is available 24 hours per day, seven days per week.
Intermediate Care Facilities/Long Term Care Nursing A facility where shelter, food and nursing care or related services areprovided for a period of more than 24 consecutive hours to persons residing atsuch facility who are ill, injured or disabled and do not require hospital orskilled nursing facility care.
Nursing Facilities/Long term Care Nursing A facility where medical care, nursing care, rehabilitation or related services and associated treatment are provided for a period of more than 24 consecutive hours to persons residing at such facility who are ill, injured or disabled.
Skilled Nursing Facilities/Long Term Care Nursing A facility where medical care, skilled nursing care, rehabilitation or related services and associated treatment are provided for a period of more than 24 consecutive hours to persons residing at such facility who are ill, injured or disabled.
