Journey Hospice provides hospice care, as well as palliative care, to individuals and their loved ones during an advanced illness or when such illness requires dedicated symptom management. These services are provided in either the patient's home, a hospital or a nursing care facility.

They also ensure a smooth transition between each care setting to alleviate a patient's stress and discomfort. The Journey Hospice care team provides comfort, love and respect, ensuring quality healthcare, and the ability to live life to its fullest, every day. Led by a Medical Director, the hospice care team includes: hospice nurses skilled in symptom assessment and pain management, social workers experienced in emotional support and planning, specially-trained home health aides, a spiritual coordinator and bereavement counselors, as well as physical, occupational, and speech therapists.
Through expert symptom management, family support, education, and access to therapies we are able to improve comfort and provide peace of mind.
Palliative care is a comprehensive treatment plan to help patients
living with a serious, chronic, or life-threatening illness manage
their symptoms more comfortably. It is implemented in tandem
with a patient’s primary medical treatment to help keep them as
comfortable as possible, and improve their quality of life.
Journey Hospice has developed a palliative care team of experts
who provide treatment for relief from: pain, shortness of breath,
nausea, vomiting, weight loss, anxiety, problems with sleep,
depression and a number of other ailments. Palliative care is
appropriate for those who have difficulty managing their care,
resulting in frequent hospital stays. Patients do not have to be
in hospice or end-of-life care to receive palliative care services.
Patients of all ages are able to receive care, whether their
medical condition can or cannot be cured.
