Types of Housing/Care Search Definitions
Adult Day Care Adult Day CareAdult Day Care services allow eligible senior adults to be in supervised group settings during day-time hours for one to five days per week. There are two types of adult day care services: Adult Medical Day Care and Adult Group Day Care. Adult Medical Day Care provides medically oriented services such as nursing, personal care, medication management, physical and occupational therapies, meals and socialization. Adult Group Day Care (often referred to as Social Day Care) provides supervised socialization with a therapeutic focus, some assistance with mobility, a meal and other activities.
Assisted Living/Residential Care Assisted Living Assisted Living and Residential Care services provide individuals and families with options that support various lifestyles of aging adults and adults with disabilities or chronic illness. These services provide support from independent living within a group setting to complete care 24 hours a day. 1. Assisted Living Services, these services are designed for adults who qualify for nursing home care and can no longer manage independent living in their own homes. Assisted living facilities provide a wide variety of support services based on the specific needs of the resident. Assisted living services may include: nursing care and personal care, nutrition and homemaker services, and medication management. 2. Congregate Housing Support Services provide housing combined with community based support services to aging adults and adults with disabilities who are residents of federally assisted housing. These services include professional service coordination in addition to non-medical support services. 3. Residential Care Services provide support for adults who qualify for care in residential settings. Residential care facilities are community-based settings that allow for maximum individual independence and are tailored to individual needs. Often these settings are appropriate for frail seniors and adults with disabilities who need more personal assistance and supervision than possible with care at home. Some of the available Adult Residential Care Services are: meal and homemaker services, personal care and individually tailored supports, adult day care and nursing care, and socialization.
Home Health Facility Home Care Home health care provider means a home health care provider as defined in RSA 151:2-b which offers, and is licensed under RSA 151:2, I(b) to offer health care services and which is a nonprofit or charitable institution or organization, public or private, which is exempt from federal taxation pursuant to section 501 of the United States Internal Revenue Code of 1986 as amended.
Hospice Hospice Hospice means a program focusing on providing terminally ill patients and their families with palliative care and social, spiritual, and counseling services.
Nursing Home Facility Nursing A Nursing Facility is an institution or facility, or a distinct part of an institution or facility, whether proprietary or non-proprietary, which is primarily engaged in providing 24-hour care for residents needing: (a) Skilled nursing care, medical monitoring, and related services; (b) Rehabilitation services for the rehabilitation of injured chronically disabled or sick; (c) Medication administration or instruction and supervision; or (d) On a regular basis, health-related care and services (above the level of room and board) which can be made available to elderly or disabled patients only through institutional facilities which provide 24-hour care.
