| Adult Day Care Centers | Adult Day Care | Adult Day Care Centers are facilities that are either operated for profit or that desire licensure. They provide supplementary care and protection during part of the day only to four or more elderly, infirm or disabled adults who reside elsewhere. The State Board of Social Services promulgates regulations for the operation of Adult Day Care Centers. Adult Day Care Centers are community-based group programs designed to meet the needs of adults with impairments through individual plans of care. These structured, comprehensive, nonresidential programs provide a variety of health, social, and related support services in a protective setting. By supporting families and other caregivers, Adult Day Services enable participants to live in the community. Adult Day Services assess the needs of participants and offer services to meet those needs. Participants attend on a planned basis. |
| Assisted Living | Assisted Living | Assisted Living Facilities (ALFs) are non-medical residential settings that provide or coordinate personal and health care services, 24-hour supervision, and assistance for the care of four or more adults who are aged, infirm or disabled. |
| Home Care Organizations | Home Care | Home Care refers to a wide variety of services provided to senior and disabled persons who may not need Nursing Facility Care, but require some assistance with their day-to-day health and personal needs while living at home. Home Care Services fall into two general categories: skilled and home support services. Skilled care is given under the general direction of a physical and occupational therapists. These services may include (but are not limited to): home dialysis; nutrition services (prescribed and adjusted diets); skilled nursing (prescribed by a physician); and therapy for specific problems such as: speech therapy, respiratory therapy (breathing), physical therapy (movement) and occupational therapy (managing household or work tasks). Home supportive services assist an individual in his or her desire to continue to live independently at home. These services include (but are not limited to): personal care (walking, bathing, dressing, assistance with eating etc); homemaker services (shopping, meal preparation, light housekeeping); chore services (minor home repairs, heavy cleaning, and yard work); and home delivered meals to those unable to prepare meals themselves. |
| Home Health Agency | Home Care | A Home Health Agency is a type of home care organization. These agencies provide nursing care and certain other health care services that an individual can have in their home for the treatment of an illness or injury. |
| Hospice | Hospice | Hospice gives care to terminally ill patients with six months or less to live. The service is licensed in the Commonwealth of Virginia not the facility. |
| Nursing Homes | Nursing | Nursing Homes provide 24 hour supervised nursing care, personal care, therapy, supervised nutrition, organized activities, social services, room, board, and laundry for elderly or disabled individuals. Often a person enters a nursing home for short-term rehabilitation, or improves and no longer needs Nursing Home care. Nursing Facility Case Management provides ongoing contact with the resident to promote a timely return home with necessary services. When 24 hour nursing care is no longer needed, Home and Community Services staff can help individuals return home or to another care setting. |
| Assisted Living | Assisted Living | The assisted living facility regulations provide for two levels of licensure, residential living care and assisted living care. The requirements for the residential living care licensure apply to those facilities that care for individuals who require only minimal assistance with the activities of daily living. |
