| Home Health Agency |
Home Care |
A Home Health Agency is an organization that is engaged in providing home health services according to a written plan of treatment signed by the patient's physician in the residence or home of the patient. These home health services include part-time or intermittent skilled nursing care and at least one of the following: physical, occupational, or speech therapy; medical social services; or home health aide services. |

Hospice |
Hospice is an organization providing a centrally administered and autonomous continuum of palliative and supportive care to persons who are terminally ill and to their families. Hospice Care is directed and coordinated by the Hospice Care team primarily in the patient's home, but also on an outpatient and short-term inpatient basis. |
| Nursing Homes |
Nursing |
Nursing Homes are facilities that admit elderly or disabled residents on medical referral and maintain services and facilities for institutional and intermediate care. (Intermediate Care means the provision of food, including special diets when required; shelter; laundry; and personal care services, such as help with dressing, getting in and out of bed, bathing, feeding, medications and similar assistance. Services are under the supervision of a nurse.) |
| Personal Care Homes |

Personal Care Homes include any dwelling that provides or arranges for the provision of housing, food service, and one or more personal services for two or more adults who are not related to the owner or administrator by blood or marriage. (Personal services include but are not limited to: individual assistance with and supervision of self-administered medications and essential activities or daily living such as eating, bathing, grooming, dressing, and toileting).

| Private Home Care Providers |
Home Care |
Private Home Care Providers are licensed by the state and provide nursing services, personal care services and/or companion sitter services to persons in their own homes. These providers are not Medicare certified but some may be accredited. These providers are inspected every other year using the Private Home Care Provider rules. Surveyors from the Office of Regulatory Services investigate complaints about care quality. |
