| Assisted Living | Assisted Living | Assisted Living Facilities provide meals, personal assistance, and assistance with activities of daily living for senior living in a home-like environment. |
| Home Health Agencies | Home Care | Both public and private HHCs provide home health care with skilled nursing care and therapeutic services (excluding the treatment of mental illness) in a patient’s home. A Home Health Care agency must provide skilled senior nursing services and at least one of the following additional services: physical therapy, occupational therapy, speech therapy; or home health aide services. |
| Hospice | Hospice | Hospice care provides physical, psychological, social and spiritual care for dying persons and their families. Services are provided by trained volunteers. Hospice also provides community education regarding loss & bereavement and direct services to bereaved persons. |
| Nursing Facilities/ Nursing Homes | Nursing | A Nursing Home is a senior facility that provides nursing care and related medical services over a period of 24 hours each day to individuals admitted because of illness, disease, or physical infirmity. Nursing Homes do not provide acute hospital care. |
