Colorado Community hospice is a team of attentive and conscientious professionals who are experts in the field of hospice care. Our goal is to support our patient’s choices, and remain responsive to our patients needs. We pledge to provide care with integrity and to be responsible stewards of the trust invested in us. We accept Medicare/Medicaid and many private insurance plans. Care is provided wherever the patient calls home. We offer palliative care services for those with a chronic disease.
